You see, there are some very interesting discussions going on about the exact nature of Vader's injuries after the Mustafar duel, and later on. There is an interesting essay on TF.N, and it is indeed very interesting, good reading and medically well-considered but I think, despite it's been updated after EpIII, it's a bit outdated. We come back at the eternal 'what is canon and what isn't' discussion now... how far can you trust what's in the books, and thus explain Vader's condition according to those. Also check the passage in which they refer to ROTJ, when you can see his skeleton because of the Emperor's Force lightning. To what degree is what we see reliable, I ask you all? To what degree is Lucas' vision reliable, at the time... many articles appeared with speculatiosn about Vader's past at the time, but that was what they were: speculations. Fill me in with your interesting opinions and speculations, your take on it or even stories.

And about Luceno's description: almost everybody agreed that it wasn't right. I haven't read the book myself but from what I heard: I tend to agree that the book has a bit an over-emo-factor-whatever.
